The level response is very much depending on the situation.
But in an ideal world it goes, advise, warn, suspend, terminate.
Each stage obviously comes with the opportunity to resolve the root cause.
On occasion it goes direct to suspend, on very rare occasion direct to terminate.
It really depends on the level of abuse ranging from "whoops I did not know" to 200+ load generated followed by a big "fuck you for suspending my DDOS as a service VPS"

I've had some abuse notices (accidentally letting a script go wild, or had some unusually big git repo which made Gitea go crazy), and all of those cases, the provider kindly stopped the VM and emailed me about high resource usage.
Since October I have had one upstream AUP complaint. Around two PayPal charge backs, both resolved respectfully.
Communication is the key.

You ensure it's a valid DMCA request and follow up with the client accordingly if you're a US based hosting provider.

Selling VPS with smtp port open is a huge pain. Every other user goes for abusing this through bulk emails. I use to control it by blocking port 25 for openvz vps but it often blow backed as legitimate users are annoyed. Second one is the credit card charge back issues. Nowdays if any user has more than one attempt with different credit card attempts on our website we simply refund them and ask to share picture of cc with numbers hidden. To my surprise non has ever responded to that request.

You usually investigate, take down copyrighted material or dispute it with valid explanation. If done within a timely manner, nothing happens.
Now it is situation like ddos/CP etc, that's a whole different ball game.

Are those charge-backs really caused by stolen credit cards or just by "frustrated" clients?

If it is frustrated client, you can dispute it with proper documentation.
Stolen cards/unauthorized transactions are where it is an instant loss.

We use 2checkout for processing credit card transactions and I don't know if their processing engine is from the time of egyptians or something else. However one chargeback complain was closed my favor as the transaction was 3ds but rest of them I don't stand a chance.
